Italy Just Became the Second Front in the OnlyFans-vs-AI-Porn War — and Its Prime Minister Is the Face of It

Onlyfans search engine, OnlyGuider, recently published the first quarterly Onlyfans analysis we’d ever run. US OnlyFans search demand was down 28.5% in Q1 2026 year-on-year — every single state, every single one of the 164 cities we track. In the same window, US AI porn search demand rose 76% over the trailing twelve months and 1,376% over three years. If Q1 holds, the US OnlyFans market shrinks from $2.63B to $1.88B in 2026 — a $750M difference, $481M off creator revenue, $97M off creator tax contribution.

Then on 5 May 2026, Italian Prime Minister Giorgia Meloni went public on X about AI-generated images of herself in lingerie circulating online — the third major Meloni deepfake news cycle in two years, after the March 2024 €100,000 lawsuit she won against Sardinian father-and-son operators and the August 2025 phica.eu scandal.

So we ran the same Q1 cut for Italy. Italy’s OnlyFans collapse is sharper than the US. Its Prime Minister has a higher absolute sexualised search exposure than Taylor Swift in America. Its AI porn market behaves identically to the US one — at one-fourteenth the scale, with the legislative window still open.

This is what the Italian numbers look like.


The Headline: Italian OnlyFans Demand Just Fell 42%

The US Q1 cut showed a 28.5% national decline. Italy is worse — by a wide margin.

Italy’s OnlyFans collapse is 13.7 percentage points steeper than the US. Q1 2026 contains the three lowest monthly OnlyFans readings in the entire 38-month series we track for Italy.

Meanwhile, the highest-growth keyword in the entire Italian dataset is “giorgia meloni nuda.” March 2026 alone hit 12,100 monthly searches — 84% above the 12-month rolling average and a 38-month high. Same month as the Prime Minister’s X statement. The news cycle and the demand curve are reinforcing each other in real time.


The Cities: Milan Loses Over Half, Rome Down 45%, Naples Down 42%

In our OnlyFans Wrapped 2025 report, Milan was the #2 European city for OnlyFans spending behind London — $58.4M in 2025, $423,256 per 10,000 residents. Twelve months later:

Milan’s −52.3% is the steepest decline of any major European city we track — worse than Memphis (−50.8%), the worst US city in our Q1 cut. The three cities that produced over $92M in 2025 OnlyFans spending have, between them, lost roughly 850,000 monthly searches in the year that followed.

Same early-mover dynamic the US data showed, sharper edge.


Italian Deepfake Demand Is Rising — and Decentralising

While OnlyFans declines, deepfake demand rose nationally:

  • Italy: +16.6% (54,300 → 63,300 monthly)
  • Milan: −11.9%
  • Rome: +15.7%
  • Naples: −6.2%

Geographic pattern is the inverse of the OnlyFans decline. Milan, the OnlyFans early adopter, is the only major Italian city where deepfake demand is falling. Rome, comparatively late on the creator-economy curve, is where deepfake is now growing fastest.

In our US Q1 cut, 140 of 164 cities (85.4%) showed OnlyFans falling and AI porn rising in the same quarter — Pearson r = −0.42, p < 0.001. The Italian pattern is the same shape: substitution happening city by city, with the geographic load shifting away from Milan into the rest of the country.


Meloni Beats Taylor Swift

This is the line that should make Italian news editors sit up.

OnlyGuider mapped Meloni’s full Italian search footprint — 22,269 keywords, 689,710 monthly searches, every Italian-language query that mentions her by name. After filtering through a wide adult-themed lexicon (Italian + English) with false-positive controls, 569 keywords are sexualised in nature, totalling 24,150 monthly searches — 3.5% of all Meloni search demand.

For comparison, here are the top five US celebrities by combined AI porn + deepfake search demand from OnlyGuider’s US celebrity exposure ranking:

The Italian Prime Minister beats Taylor Swift in absolute terms. On a per-capita basis she beats Swift by 5.9 to 1. A sitting head of government is generating sexualised search demand in her own country at almost six times the rate Taylor Swift generates in the United States.

The 9.1% feet-fetish concentration is unusually high for a head of government. The most plausible explanation is the BBC-reported €100,000 lawsuit Meloni won in March 2024, which featured deepfake content viewed millions of times. Two years later the Italian search market has functionally trained itself to associate the Prime Minister with that specific niche.

The synthetic-only volume is 870 monthly searches across 50 keywords like “giorgia meloni deepfake” (260) and “giorgia meloni fake porn” (140). That looks small in isolation. It isn’t isolated. Every search ending in “meloni nude / nuda / hot / feet / bikini” is functionally a deepfake-adjacent query, because no real such content exists. The full 24,150 are, in practice, demand for synthetic content of the Italian Prime Minister.


What This Means for Italian Spend, Creators and Tax in 2026

In our US Q1 cut, we put a number on it: $750M off US OnlyFans spend, $481M off creator revenue, $97M off creator tax contribution if Q1 2026 holds for the rest of the year.

The Italian numbers are smaller in absolute terms — but the percentage drop is sharper, so the proportional hit is bigger. Our  OnlyFans Wrapped 2025 report recorded $354.8M in Italian fan spending in 2025 (+24.5% YoY), supporting 85,053 active creators who collectively earned $110.5M gross.

If the 42.2% Italian OnlyFans search collapse translates proportionally into spend behaviour through the rest of 2026:

For 85,053 Italian creators: an average gross revenue cut of roughly $540 per creator if the trend holds. That hits the long tail hardest — the top 10 creators in our list will absorb a 42% revenue drop and still earn six figures. The bottom 80% of the creator base may not.

For Milan specifically: the city that built itself into the second-largest OnlyFans market in Europe behind London could lose more than half its spend in twelve months. The Milan creator economy was $58.4M in 2025. The mechanical 2026 number is closer to $28M.

For the Agenzia delle Entrate: the €19.6M “ethical tax” pot calculated against 2025 revenues is a moving target. If 2026 search-to-spend correlation holds, the pot may be closer to €11.4M.

Three caveats — important. Search demand and platform spend are correlated, not identical. Existing subscribers may keep paying without searching. The trend is recent and may stabilise — or accelerate. And Italian users rotating into deepfake and AI porn searches are not, on the whole, paying the same per-user rates the OnlyFans subscription model commands. These are illustrative figures, not forecasts.


The 18% CREATE-Intent Finding — And Why It Matters

In OnlyGuider’s US Q1 2026 Onlyfans report, the AI porn market wasn’t just bigger than the OnlyFans market in some places — it was structurally different. Roughly one in five US AI porn searches is CREATE-intent: users actively trying to manufacture synthetic adult imagery, not consume it. Generators, makers, undress tools, face-swap utilities.

We ran the same intent decomposition for Italy. The result is “almost” identical.

Italian users are doing the same thing as American users, in the same proportions, at one-fourteenth the scale. The Italian AI porn market is not an early or different version of the US market. It is the same market, smaller. The only thing Italy has that the US has lost is time.

The Italian deepfake corpus tells the parallel story: of 214,060 monthly Italian deepfake searches, 7.2% target named individuals — celebrities and politicians, 15,430 monthly searches. Just 0.4% are about law, policy or regulation. Italian public discourse about regulation is being outweighed by demand for the underlying content by roughly 16 to 1.

The Italian Prime Minister is, right now, the most-searched named target inside that 7.2% bucket. She is the visible top of an iceberg that the rest of Italian public life sits underneath.


The Headache to come

Italy’s OnlyFans search demand fell 42% in twelve months — steeper than the US 28.5% decline that prompted OnlyGuider’s Q1 2026 US report two weeks ago — its Prime Minister now has higher absolute sexualised search exposure than Taylor Swift in America, and 18% of all Italian AI porn searches are users trying to manufacture synthetic adult imagery: the same intent profile as the US market that is fourteen times its size.

Italy is the second front. Its Prime Minister is the public face. The legislative battle the US and Italy have against AI Porn is going to be a massive topic of debate over the next few years as AI advances and adoption increases.

Methodology

Trend data: Google Ads Keyword Planner exact-match monthly search volume, pulled May 2026 for Italy national and Milan / Rome / Naples city geos. 38-month series (Jan 2023 – Mar 2026). Q1 averages = arithmetic mean of January, February and March. YoY change = (Q1 2026 − Q1 2025) / Q1 2025.

Keyword selection — important caveat. The Google Ads API used to pull Italian trend data blocks the phrase “ai porn” in the Italian market and returns no monthly volume for it, so a direct “OnlyFans vs AI porn” trend comparison equivalent to OnlyGuider’s US Q1 2026 cut was not possible for Italy via this data source. The trend curves in this report use “deepfake” as the closest available synonym for AI-generated adult content in the Italian market, and “giorgia meloni nuda” as the highest-volume named-individual deepfake-adjacent query — both of which the API does return at city and national level. The structural intent, volume and niche analysis (the 18% CREATE-intent finding, the AI porn / deepfake / OnlyFans intent breakdowns, the 569-keyword Meloni adult corpus) uses Semrush broad-match data, which does return Italian AI porn keywords.
