Milan has 6,023 creators — over 1.6x more than Rome despite having a smaller population. Its creator density of 44 per 10,000 residents reflects its status as Italy’s media, fashion, and entertainment capital. Rome’s 3,654 creators at 13 per 10k speaks to a more dispersed creative base across a sprawling capital. Naples, with 817 creators, is a smaller but meaningful market.

Milan’s numbers are globally elite. With $423,256 spent per 10,000 residents, Milan ranked #1 in all of Europe and #3 globally for OnlyFans spend intensity in 2025. Its $58.4 million in total spend makes it the #2 European city by volume, behind only London.

Rome generated $26.2 million in total spend (+22.3% YoY), ranking #4 in Europe by city spend. The Ethical Tax ? — What Italian Earnings Mean in Practice

Italy’s OnlyFans economy has attracted the attention of the Agenzia delle Entrate (Italian Revenue Agency). With $110.5 million in gross creator revenue flowing annually, the platform has moved well beyond a niche side-hustle into a significant part of the Italian gig economy.

National-Level Tax Breakdown

Based on OnlyGuider’s revenue data, accounting for OnlyFans’ standard 20% platform fee and EUR/USD conversion at the 2-year average rate of 0.886799:

The ‘Ethical Tax’ Debate

Italy’s Tassa Etica — a 25% surcharge on income from the production of pornographic material — was introduced in 2005 for the traditional adult entertainment industry. For most of its existence it was largely ignored in the context of digital creators. That is no longer the case.

Of Italy’s ~85,000 OnlyFans creators, roughly two groups exist from a tax perspective. Around 40,000 operate under Italy’s simplified or ordinary tax regimes — for them, the ethical tax has always unambiguously applied, since it is legally defined as an addizionale (surcharge) on top of standard income tax (IRPEF).

The remaining ~45,000 creators operate under Italy’s forfettario (flat-rate) regime — a simplified system where a single flat tax of 5% or 15% replaces income tax entirely. Because the ethical tax is technically a surcharge on income tax, many argued it couldn’t attach to forfettario taxpayers at all, since there was no underlying income tax for it to sit on top of. That ambiguity gave tens of thousands of creators a potential legal shield.

In November 2025, the Revenue Agency closed that debate. Responding to a formal query from fintech company Fiscozen, the Agenzia delle Entrate confirmed the ethical tax applies to forfettario taxpayers too. The last legal grey area is gone.

The full ~85,000 Italian creators are now unambiguously in scope — assuming their content meets the regulatory definition of pornography (“explicit, unsimulated sexual acts between consenting adults”). Content such as feet photos, simple nudity, or non-explicit fetish material may still not qualify, assessed case by case.

The Italian Radicals have launched the campaign L’Etica Non Si Tassa (“Ethics Should Not Be Taxed”), arguing the levy is disproportionate and risks incentivising tax evasion in an increasingly professionalised creator community.

Methodology

Creator data — 2026 Global Creator Census: OnlyGuider audited over 300,000 OnlyFans creator profiles using an AI engine deploying NLP and Computer Vision to identify regional linguistic markers, localised metadata, and cross-platform geographical indicators. Creator counts were reconciled against Fenix International Ltd’s audited corporate creator count figures. Revenue was modelled by distributing audited global revenue ($7.2bn, 2025) across the identified regional creator workforce.

Spending data — OnlyFans Wrapped 2025: OnlyGuider harvested over 2 years of monthly search volumes for “OnlyFans” across 216 countries and 300+ cities using the Google Ads API, weighted by conversion quality using a proprietary Revenue Per Search (RPS) model, anchored to Fenix International’s audited UK Companies House filings ($6.6bn in 2024; $7.2bn in 2025). City-level spend was derived by applying country-specific RPS to city search volumes.

Creator earnings estimates: Individual creator earnings are estimated using annual search volume as a proxy for audience engagement, calibrated against OnlyGuider’s platform revenue data and the audited global figure.
